The Medical Examination
Upon successfully completing registration and payment, you will receive a confirmation email containing important instructions and links. The email includes links to download and upload the Medical Exam Form.

The Medical Exam Form required is the same as the Scouting America ABC. If you possess a current form (less than a year old at the time of the session), you may submit that. All completed medical exam forms must be submitted by May 31st. The forms can be found on the last pages of the Session Information Packet. Parts A, B1 & B2 are filled out by parents/guardians. Part C is completed by a medical professional.

To streamline the process, don't wait to schedule your appointment. We recommend printing the form, scheduling a well-child exam, and bringing Part C along to be filled out and signed by the doctor during the appointment. This ensures timely completion and submission of the required medical documentation.
