The Medical Examination and Facial Snapshot

Upon successfully completing registration and payment, you will receive a confirmation email containing important instructions and links. The email includes links to download and upload the Medical Exam Form, as well as a Facial Photo snapshot of the participant.

The Medical Exam Form required is the same as the BSA Medical Form Part C. If you possess a current form (less than a year old at the time of the session), you may submit that. All completed medical exam forms must be submitted by May 31st. The form can be found on the last page of the Session Information Packet.

To streamline the process, we recommend printing the form, scheduling a well-child exam, and bringing the form along to be filled out and signed by the doctor during the appointment. This ensures timely completion and submission of the required medical documentation.
